Official Rules
No purchase necessary to win; open to all U.S. Citizens 18 or older. Total approximate prize value of up to $4,999.00 There will be no substitute for the above prizes, cash or otherwise. Taxes, or other fees, are the responsibility of the winner. All entries become property of MedAware, Inc. Only one entry per household. Only one entry per envelope. Mutilated or illegible entries, and mass, or reproduced entries, will be disqualified. Winners will be required to sign an affidavit of eligibility and in compliance with the rules and must agree to the use of their name and photograph for publicity purposes. The decision of the judges awarding the prizes shall be final. One winner will be chosen at random on September 7th, 2007. All entries must be postmarked no later than August 31, 2007 and received no later than September 6th, 2007. The winner will be notified by phone and mail.
